ASK
Ask each of your employees if they have been fully vaccinated (defined as 2 weeks past the 2nd vaccination for Moderna and Pfizer, or 2 wks past the J&J single dose)
Fully remote workers need not submit this information
Vendors working for you on-site must request this documentation from their staff
Any answer from your employee is acceptable for the sake of your record-keeping, you do not need to request proof of vaccination
For your records, you need only document “vaccinated” or “unvaccinated”
If your employee declines to state, they fall under the “unvaccinated” status
Circle back with unvaccinated employees every 14 days so you are able to document any change in status
Please note, there may be additional requirements mandated for employees who do not elect to get vaccinated
